One goal of the Georgia Consortium for Fieldwork Education is to provide resources for fieldwork coordinators and educators to utilize during program development and/or direct supervision of fieldwork students.

Created by the Brenau OT Fieldwork Team and presented at the 2018 GOTA Conference. This presentation addresses: 

  1. Fieldwork placement process. 

  2. FERPA, ADA, and other legislation that directly affect fieldwork programs.

Weekly Conference Form

Created by the Brenau University Fieldwork team, this sample resource is a great way to have weekly discussion to ensure students clearly understand their progress (positive and negative), as well as future expectations.

Level II Suggested Progression of Expectations

Created by the Brenau University Fieldwork team, this resource serves as a general outline for the progression of expectations for a level II student over the course of a 12-week fieldwork rotation. This resource can be modified to suit the needs of the student, fieldwork educator, and facility.

Personal Data Sheet

The Personal Data Sheet developed by AOTA is a great tool to gather as much information about the student as possible in order to share with the Fieldwork Educator.

Fieldwork Education Assessment Tool (FEAT)

The FEAT (courtesy of AOTA) identifies essential characteristics of the three key components. By providing a framework to explore the fieldwork experience, the FEAT can help students and fieldwork educators consider how to promote the best possible learning experience.


The purpose of the FEAT is to contribute to student and fieldwork educator discussions, so that reflection and
problem solving can occur to enhance the fieldwork experience. The tool is designed to both assess the balance of the
three key components, and to facilitate discussion about student and fieldwork educator behaviors and attitudes, and
environmental resources and challenges. By mutually identifying issues present during fieldwork, the fieldwork
educator and student can use the FEAT as a tool to promote dialogue and foster the identification of strategies to
facilitate the just-right challenge. The FEAT may be used early in fieldwork as a tool to promote dialogue, or at
anytime throughout fieldwork as the need for problem solving emerges.

Site Specific Objectives

Fieldwork is completed in a wide variety of practice settings. Creating site-specific objectives is a great way to clearly communicate the expectations of your particular facility/practice area. Use the link above for a variety of samples from AOTA.

AOTA Fieldwork Data Form

Accredited OT and OTA Programs are required to maintain updated AOTA Fieldwork Data Forms on all of their Fieldwork Sites. While lengthy, this form can help the Fieldwork Educator and Academic Fieldwork Coordinator have a detailed discussion on the nuances of the facility and practice area.
